Therapy Success Story

Concord loves to celebrate our residents and their achievements. Being a skilled nursing facility, our Therapy Department stands out for its diverse team that is culturally advanced in handling resident, staff and family needs. We actively pursue excellence and empower our employees to be the reason we make a difference in the lives we serve.

We want to celebrate one of our residents, Thelma H., for meeting her therapy goals! Upon admission in November of 2021, Thelma was evaluated and found to be at a dependent level with moderate cognitive issues and decreased function in all areas. She was initially expected to be a long-term patient due to her condition.



After a couple of weeks, she started meeting some of her short-term goals. Once a month had passed, she showed some potential for being able to go home. Thelma worked very hard, and after not being able to sit herself up in bed or have a conversation, she is now able to return home with sound cognition. She is now independent with all mobility and the use of her rollator walker. After meeting these goals, Thelma will return home, where she lives with her sister, at a safe and independent level.

Congratulations, Thelma! Our team is so happy to have been able to help you regain your strength so you can return home to your family.
